In the case of an emergency, please call 911. We are available twenty-four hours a day, seven days a week to meet our patients' needs. Please call any one of our offices listed below and our answering service will contact our on-call physician who will get back to you as soon as possible.
Appointments
Office visits are by appointment and are easily made by calling any one of our offices. We understand special needs and we make every effort to accommodate same day appointments. We use an automated system to remind you by telephone of upcoming scheduled appointment. You will receive a call 1 to 2 days prior to your appointment.
At your appointment, please update any changes in name, address, telephone, or cell phone. Please bring the following to your appointment:
- Your insurance card
- A list of current prescription or over-the-counter medications you are taking, including dose and frequency
- Your medical and surgical history
- X-rays or records you may have
- Physician referral forms if required by insurance
Medical information and medical records are strictly confidential. No information will be released to attorneys, insurance companies, or any other third party without your written authorization.

Insurance and Payment Information
Pensacola Urology is a provider for Medicare and most major insurance plans. Insurance companies are responsible to you, the policy holder, not to the doctor. Please check your insurance providers' list of participating physicians to be sure your plan allows you to be seen at our office. Also, please review with your insurance provider the rules and limitations of coverage for your particular plan. Coverage decisions are between you and your insurance provider, as a courtesy we do provide insurance billing.
